Thermage Eye: Can Radiofrequency Help Hooded Eyelids?

Thermage Eye is a non-surgical radiofrequency treatment for the upper and lower eyelids that can modestly tighten crepey skin and soften mild hooding. It uses a small dedicated eye tip to heat the delicate eyelid skin and stimulate new collagen over several months. It suits mild skin laxity, but it cannot remove excess skin or fat the way eyelid surgery does.
What is Thermage Eye?
Thermage Eye uses the same Thermage FLX system as facial treatment, with a much smaller tip designed for the thin skin around the eyes. Monopolar radiofrequency (RF) energy passes into the skin and turns into heat in the dermis. The heat contracts existing collagen and starts a healing response that builds new collagen over two to six months.
In the United States, Thermage has been cleared for the non-invasive treatment of wrinkles around the eyes, including the upper and lower eyelids. Regulatory approvals differ between countries, so ask your clinic about the local status.
Can Thermage Eye help hooded eyelids?
It depends on what is causing the hooding.
- Mild skin laxity: if the upper lid skin has become slightly loose and crepey, Thermage Eye can tighten it a little and may open the eye area subtly.
- Excess skin (dermatochalasis): if there is a significant fold of extra skin resting on the lashes, RF is unlikely to give a satisfying result. Surgery (blepharoplasty) is usually more effective.
- Brow descent: some "hooding" is actually a lowered brow. Focused ultrasound brow lifting or other options may suit this better, sometimes alongside RF.
- Fat bulging or puffiness: RF does not remove fat. Puffiness caused by fat pads or fluid needs a different approach.
A doctor can usually tell which of these is the main cause during an examination.
What happens during Thermage Eye treatment
- Assessment: the doctor examines the eyelids, brows and under-eye area and agrees the number of shots. Eye treatments commonly use around 225 to 450 shots.
- Numbing: a topical numbing cream is applied to the eyelids. For upper and lower lid treatment, anaesthetic eye drops are used and protective eye shields are placed over the eyeballs.
- Treatment: the small tip is moved over the lids in passes, with cooling before and after each pulse. You feel short bursts of warmth.
- Finishing: the shields are removed, the eyes are rinsed if needed, and you can usually leave shortly afterwards.
The shields can feel strange, but they protect the eye from the energy. This step should only be done by trained medical staff.
Recovery and side effects
Downtime is usually minimal. Common, temporary effects include:
- Mild redness and swelling of the lids for a few hours to a couple of days.
- Slight irritation or a gritty feeling in the eyes after the shields, which usually settles quickly.
- Tenderness when touching the area.
Less common risks include blistering, burns or prolonged eye irritation. Contact your clinic if you have eye pain, blurred vision that does not clear, or skin changes that worsen instead of improving.
Results timeline
- Immediately: some people notice slightly smoother, firmer lids.
- 1 to 3 months: gradual tightening of crepey skin as collagen builds.
- Around 6 months: results usually reach their peak.
- Duration: often around a year, varying with age and skin quality. Many people repeat annually.
Who is a good candidate for Thermage Eye?
- Adults with mild upper lid laxity, crepey lids or fine lines around the eyes.
- People who are not ready for surgery or want to delay it.
- Those who want a subtle result with little downtime.
It may not suit people with marked excess skin, eye conditions such as active infection or recent eye surgery, implanted electronic devices, or those who are pregnant. Tell your doctor about contact lenses, dry eye and previous eye procedures.
Aftercare
- Avoid rubbing your eyes for a day or two.
- Skip contact lenses on the day of treatment unless your doctor says otherwise.
- Use a gentle, fragrance-free eye cream and daily sunscreen around the eye area.
- Avoid saunas and intense exercise for one to two days.
Frequently asked questions
Is Thermage Eye painful?
With numbing cream and anaesthetic drops, most people find it tolerable. You feel warm pulses rather than sharp pain.
Can Thermage Eye replace eyelid surgery?
No. It can improve mild laxity, but it cannot remove excess skin or fat. For heavy hooding, surgery is usually more effective.
Can I combine Thermage Eye with full-face Thermage?
Yes. Many people treat the face and eyes in the same session, using the appropriate tip for each area.
How many sessions do I need?
Usually one session, with results reviewed at around six months. Some people repeat yearly to maintain the effect.
